SPINA - Study Registration Page

Welcome to the SPINA Spinal Rehabilitation Registry!

Please enter your information in all the fields provided. This information is necessary for SPINA to accurately record your information and send any instructions or reminders.

Once you enter your information you will receive an email with instructions on how to log onto the SPINA registry. Once you log on, you will be presented with a detailed letter of information describing the SPINA registry and you will be invited to read and asked to sign a consent form.

The information you enter here will only be stored if you sign the consent form. Registration information is updated weekly and any information you have entered here will be deleted if you decide that you do not wish to sign the consent form.

If you are entering the registry through a physiotherapy clinician as you undergo current treatment, then please select the name of your physiotherapy center and your physiotherapy clinician.

If you are not entering the registry through a physiotherapy clinician, or if you do not have a physiotherapy clinician, you are still welcome to join! Please keep the default setting of "I am not entering through a physiotherapy clinician" in the CENTER and CLINICIAN boxes.
